we were informed that SOAP format should not be used anymore. As mentioned by one of our senior dietitian in HKL, it was "zaman pak kadok" format. huhu.. So, suppose to use nutrition care process (ADIME). We've learnt this theoretically right. But not so much in practical. So, here's an example:
